I declare that I am a woman, a citizen of Nepal and aged 18 or above, and that everything in this form and my video is true.

I understand that this form and video are my first audition, and that only 36 women are selected in each province.

I understand that if selected for the provincial round I must sign the Participant Agreement and pay a Provincial Training Fee of NPR 45,000; that the National Program Fee for finalists is NPR 108,000 in total with the NPR 45,000 deducted; and that I use NPR 12,000 of my own money on Market Day.

I allow VUEX Global and its partners to check my records and use my video, name and photograph for selection, television and publicity. I accept that the selection panel’s decision is final.
